League of Legends (LoL) Question: Can't verify my username?

Can't verify my username?
I've made a guide not too long ago and I just can't seem to get my rank to show up on my it due to my misunderstanding on how I can verify my username. Also I want to state that there is no image displayed on the verification page or I am missing something. Any help would be appreciated!

I've checked your profile and I think you haven't included your complete summoner name. You also have to include the # tagline for the verification to work.
